  • Para diagnosticar el TDAH se recomienda identificar y valorar los síntomas (DSM-5 y/o CIE-10), hacer un diagnóstico diferencial y una evaluación clínica, psicosocial y pedagógica del paciente.
  • En todos los pacientes con TDAH se recomienda terapia no farmacológica.
  • La utilización de tratamiento farmacológico junto con la terapia no farmacológica se recomienda en función de la edad, el grado de afectación y el rechazo/ineficacia del tratamiento no farmacológico.
  • El grado de afectación del paciente se mide por la interferencia que supone la sintomatología en el desfase entre su rendimiento y su capacidad real.
  • Antes de iniciar el tratamiento farmacológico, hacer una valoración del paciente para seleccionar fármaco, dosis, pauta y formulación.
  • Los fármacos psicoestimulantes se recomiendan como tratamiento de primera elección en niños/adolescentes salvo contraindicación: metilfenidato como primera opción y lisdexanfetamina como alternativa.
  • Los fármacos no psicoestimulantes se recomiendan como alternativa a los psicoestimulantes en niños/adolescentes en caso de ineficacia, contraindicación o intolerancia: atomoxetina como primera opción y guanfacina como alternativa.
  • Atomoxetina es el único fármaco autorizado para el tratamiento del TDAH en adultos, pero metilfenidato y lisdexanfetamina podrían continuarse en >18 años que lo precisen. Guanfacina no debe utilizarse.
  • Ajustar el tratamiento farmacológico individualizadamente (período de prueba 4-6 semanas) hasta la dosis óptima o la máxima tolerada; y planificar un seguimiento para valorar la continuación, modificación o suspensión del tratamiento o la necesidad de consultas especializadas.

 
  • When making an ADHD diagnosis, it is advisable to (i) identify and evaluate the symptoms (DSM-5 and/or CIE-10); (ii) conduct a differential diagnosis; and (iii) conduct a clinical, psychosocial and school-based assessment of the patient.
  • Non-pharmacological therapy is recommended to all patients diagnosed with ADHD.
  • Recommendations for prescribing the combination of non-pharmacological and pharmacological therapy should be based on (i) the age of the patient; (ii) its level of impairment, and (iii) refusal or inefficacy of non-pharmacological interventions.
  • The level of impairment of a patient is measured by the interference of the symptomatology in the mismatch between the patient’s performance and his/her true capabilities.
  • Before initiating pharmacological treatment, it is important to make an assessment of the patient in order to select the appropriate medication, dose, dosage regimen and dosage form.
  • Psychostimulant medication is recommended as the first-line choice for children and adolescents, unless contraindicated. Methylphenidate should be the first choice of treatment and lisdexamfetamine the alternative option.
  • Non-psychostimulant medication is recommended as an alternative option in children and adolescents when psychostimulants fail to be effective, are contraindicated or poorly tolerated. Atomoxetine should be first option and guanfacine the alternative option.
  • Atomoxetine is the only drug licensed for the treatment of ADHD in adults. However, methylphenidate and lisdexamfetamine may be continued in patients aged 18 years and over, when required. Guanfacin must not be used.
  • Adjust pharmacological treatment on an individual basis (4 to 6-week trial period) to the optimal or maximum tolerated dose. A follow-up should be scheduled to assess whether to continue, change or stop treatment, or to provide specialist consultation when required.
